General Data Protection Regulation (GDPR)
This information sheet is to make you aware that in order to comply with the GDPR, I need your consent to hold your personal data for the general administration of my practice and to enable me to use the full range of communications with you.
I will not share your data with any outside organisation or person without your consent in writing unless required by law to do so.
Equally, I do not pass your information onto any other organisation or person.
You have the right to have your details removed/destroyed at any time and to see copies of the information held on you.
Reasons for holding your data
Should we start working together therapeutically, I require personal information (your data) to communicate with you either by email, post or telephone. I also require your general practitioner details for the duration of contact.
If you wish to continue to receive communications from me I will also require your consent. A signed copy of this statement will form part of the therapeutic agreement at the commencement of therapy.
